COMMENTS: There were 122 permanent care home admissions in quarter 4. This is expected. Although the domiciliary care market position is improving we are seeing a slightly higher than normal number of admissions to care homes.

COMMENTS: National guidance is that all care plans should be reviewed at least every twelve months. Work continues in this area to assist in improvements in this metric.

COMMENTS: We have a programme of working with providers to set up more independent living services which supports the outturn on this indicator.

COMMENTS: The outturn is based on 44 out of 989 service users aged 18-64, with a learning disability known to adult social care in paid employment - often complex cases.

COMMENTS: Eligible clients have the option to spend their personal budget on any support or service agreed to help them achieve their goals identified as part of their care needs assessment. Work is ongoing in relation to actions which have been identified as part of the strategic change project to improve the take-up of Direct Payments in Northumberland.

COMMENTS: This indicator reports all Northumberland based service providers rated on the CQC website. 13 providers rated Outstanding, 108 rated as Good, 19 Require Improvement and 1 Inadequate. The Adult Services Commissioning team are continuing a programme of service monitoring focused on supporting services to improve or retain a positive CQC rating. One care home has been rated Inadequate by CQC in a report published 29.3.24. CQC visit was actually prompted by a visit from the NCC Contracts and Commissioning Team, Adult Social Care. Admissions are being monitored and the home will be supported by the Contracts and Commissioning Team on the improvement action plan.

COMMENTS: Provisional outturn until completion of the year end return. We do perform well on this ASCOF indicator which demonstrates the effectiveness of short term services within Northumberland. Latest published figures report 81.7 for the North East and 77.5 for England, and Northumberland ranks 17th overall out of 151 local authorities.

COMMENTS: This outturn relates to discharges from hospital between October and December 2023. Latest published figures report 83.1 for the North East and 82.3 for England. Northumberland is ranked 25th out of 151 local authorities. Which is in the upper quartile.

COMMENTS: 2024 provisional figure from the annual Adult Social Care User Survey. Surveys were sent out to over 1400 service users in February 2024. The value is based on the response to several questions within the national survey. Higher scores are assigned to better outcomes, so the higher the overall score the better the average social care-related quality of life. The maximum possible score is 24. The latest published England average = 19.0
